Presentation

Left-sided abducens nerve palsy with ipsilateral retro-orbital pain.

Patient Data

Age: 6 years
Gender: Male

The left petrous apex shows an intermediate to low signal on T1, high signal on T2, FLAIR fat sat, and DWI with peripheral irregular enhancement. Associated middle ear fluid and partial opacification of the mastoid air cells.

Thickening of the dura of the ipsilateral Meckel's cave and cavernous sinus.

Case Discussion

The MRI features are most consistent with petrous apicitis resulting in Gradenigo syndrome.
