Presentation
History of knee pain and swelling.
Patient Data
MRI shows abundant effusion in the quadricipital, and patellar recesses associated with diffuse synovial hypertrophy characterised by multiple villous lobulated proliferation.
Oedema of the femoral and tibial bone marrow.
Effusion also occurs intraarticular and in the posterior recess of the knee.
There is a small Baker's cyst.
ACL and PCL are unremarkable.
Case Discussion
MRI findings suggest pigmented villonodular synovitis (PVS) of the knee.
The lack of the FFE sequence means that it is impossible to assess the possible presence of haemosiderin, which is typical in PVS and would have allowed greater confidence in the correct diagnosis.
