Pneumocystis jirovecii pneumonia in HIV patients

Diagnosis probable

Presentation

Low grade fever for 3 months, short of breath for 20 days

Patient Data

Age: 45 years
Gender: Male

Widespread patchy ground glass opacity.

Upper zone predominant areas of reticular opacity and cysts.

No effusions.

After 3 months treatment

Partial clearing of the lungs. Residual areas of ground-glass opacity, cysts and fibrosis.

Case Discussion

The patient was diagnosed with HIV after admission, and the CD4+ T-cell count was only 16/µl. CT was repeated after 3 months treatment with anti-HIV and sulphonamides.

Pneumocystis jirovecii pneumonia is a almost exclusively seen in immunocompromised individuals and in this clinical context these CT appearances point to this diagnosis.
