CT scan demonstrates:
Interruption of the infrahepatic segment of the IVC with hemiazygos continuation
the right renal vein is drained into the left-sided IVC as well as the left renal vein
The post-hepatic segment of the IVC is present and drains only the hepatic veins into the right atrium
Case Discussion
Polysplenia syndrome, also known as left isomerism, is a type of heterotaxy syndrome where there are multiple spleens congenitally as part of left-sided isomerism. Usually associated with congenital heart disease, malrotation of the abdominal viscera and vascular anomalies (such as intrahepatic IVC interruption with azygos/hemiazygos continuation and preduodenal portal vein)
