Presentation

Chronic locked posterior shoulder dislocation.

Patient Data

Age: 25 years
Gender: Male

Arthroscopic posterior bone block stabilization. These radiographs show the shoulder after transferring a bone block from the iliac crest to the back of the glenoid, fixed with two screws.

CT reveals a well-positioned bone-block (subequatorial and flush). The bone block's overhang is responsible for its partial resorption.

Arthroscopic iliac crest tricortical bone graft harvesting applied to the posterior glenoid bony loss.

Case Discussion

Posterior glenohumeral instability represents only 4% of all shoulder instability. The posterior bone block is one of the surgical procedures proposed in the literature for the treatment of posterior instability of the shoulder. Prognosis is better in traumatic than in atraumatic instability. Posterior bone-block techniques involve either an iliac bone-block harvested preoperatively from the endopelvic face of the ipsilateral iliac wing or an acromial block with a deltoid flap pedicle.
