Posterior nutcracker phenomenon - retroaortic left renal vein draining into IVC

Right lower quadrant pain investigated for suspected acute appendicitis.

The CT scan showed features of acute appendicitis.
However, there was an incidental note of narrow retroaortic left renal vein draining into the IVC (RLRV).
Note the splenorenal collaterals with the engorged left ovarian vein along the course of the left ureter.
Case Discussion
The patient had no retroaortic left renal vein related symptoms or lab findings, hence the nutcracker phenomenon rather than a syndrome.
