Rathke cleft cyst - T2 hypointense

Case contributed by Dalia Ibrahim
Diagnosis almost certain

Presentation

Headache. Finding on MRI of the brain dedicated MRI of the sella.

Patient Data

Age: 25 years
Gender: Male

A well circumscribed intrasellar nodule is located at the midline between both pituitary lobes. It demonstrates a homogeneous hypointense signal on T2 WI and a hyperintense signal on T1 WI. No contrast enhancement (evident on the last contrast subtraction series). No deviation of the pituitary stalk.

Case Discussion

The patient had a stable size and signal on a 1 year follow up MRI after the initial MRI sell exam.

Rathke cleft cysts are typically located at the midline between the anterior and the posterior pituitary lobes.

The signal intensities of RCCs are variable and mainly depend on their content. Most of the RCCs demonstrate T2 high signal intensity and appear non-enhancing on post-contrast series. However, RCCs may demonstrate a homogeneous T2 hypointense signal intensity.
