Renal cystic mass - Bosniak class IV

Diagnosis almost certain

Presentation

Right flank pain and microscopic hematuria.

Patient Data

Age: 70 years
Gender: Male

A 48 mm partially exophytic cystic mass with a thick enhancing wall and septal and eccentric enhancing nodules is noted in the middle portion of the right kidney. A few small dilated collateral vessels are pointed out in perinephric space.

Additionally, several simple cortical cysts are seen in the kidneys.

A few small cysts are seen in the liver. (correlated with ultrasound finding)

A 30 mm diverticulum is noted at the medial aspect of D2.

Case Discussion

Regarding enhancing nodules the right renal cystic mass is consistent with a Bosniak class IV lesion. Up to 90% of Bosniak class IV masses are malignant, and surgical excision is needed.
