Presentation

Abdominal pain. Perforation?

Patient Data

Age: 80 years
Gender: Male
x-ray

AP chest

Sternotomy and CABG. Pacemaker.

Lungs clear. No pneumoperitoneum.

Abdomen

Linear high density projected over the epigastrium–looks like a wire. This is consistent with a retained pacing wire.

Normal bowel gas pattern.

A prior CT demonstrates a retained pacing wire in the upper abdomen.

Case Discussion

Pacing wires on occasion get retained after cardiothoracic surgery. The main issue is with identification prior an MRI study.

They can be subtle so this is what you are looking for.
