Retroaortic left renal vein - type II

Presentation

Patient with recurrent attacks of left loin pain and microscopic hematuria. No renal or urinary bladder stones. A contrast study was requested.

Age: 30 years

Abnormal course of the left renal vein having a retro-aortic pathway and drains into the IVC opposite the level of L4. Finding consistent with retro-aortic left renal vein (RLRV) type II. The vein is well opacified with no evidence of compression between the aorta and the vertebral body. Normal appearance of the left kidney.

Case Discussion

The retroaortic left renal vein (RLRV) has four subtypes:

  1. RLRV joining the IVC in the normal position.
  2. RLRV joining the IVC at the level of L4–5.
  3. Circumaortic or collar left renal vein, having two limbs crossing anterior and behind the aorta.
  4. RLRV drains into the left common iliac vein.

The most common urological symptom attributed to the abnormal course of the left renal vein is hematuria.
