Sessile osteochondroma - humerus

Presentation

Palpable painless lump of the left upper arm.

Patient Data

Age: 10 years
Gender: Female
x-ray

A sessile bony mass arising from the proximal metaphyso-diaphyseal region of the left humerus, displacing the overlying soft tissues with no periosteal reaction seen.

 

The sessile bony mass arises from the posterior aspect of the proximal humeral metaphyso-diaphysis. The medullary cavity is in continuity with that of the parent bone as well as the cortex with no fracture or periosteal reaction seen. The cartilage cap is 2 mm in the thickness. Subcortical oedema with enhancement within the bony lesion. No oedema or abnormal enhancement of the adjacent surrounding tissue.

Plain film and MRI features are of a sessile osteochondroma

MRI is the best imaging modality to assess cartilage thickness. A cartilage cap of over 1.5 cm in thickness is suspicious for malignant degeneration.
