Spermatic cord trauma

Diagnosis almost certain

Presentation

Blunt trauma to scrotum 2 days previously. Persistent pain and swelling.

Patient Data

Age: 14 years
Gender: Male
ultrasound

Both testes are normal. The left spermatic cord is tortuous with surrounding fluid and inflammatory change in the surrounding fat. This extends from the level of the superior pole of the testicle to the mid-point of the inguinal canal. No other injury.

Case Discussion

Appearances were highly suggestive of an injury to the spermatic cord.

Pain and swelling resolved after 1-2 weeks.

Follow up US at 3 months was completely normal. No residual abnormality in the left inguinal region and normal cord. The previous tortuosity had resolved and there was no varicocele. Both testes were normal in size, shape and outline.
