Presentation

18 month history of worsening knee pain, giving way and locking – worse after he twisting his knee

Patient Data

Age: 60 years
Gender: Male

There is flattening of the articular surface of medial femoral condyle with volume loss and marrow oedema.

There are tricompartmental degenerative changes of knee with degenerative horizontal signal of medial meniscus. The ligaments appear intact.

Spontaneous osteonecrosis of the knee (SONK) also known as subchondral insufficiency fracture of the knee is a type of stress fracture involving the femoral condyles or tibial plateau occurring in the absence of trauma and typically affecting older adults.

Differentials include osteochondritis dissecans, which usually affect children and young adults.
