Sporadic Creutzfeld-Jakob disease

Diagnosis almost certain

Presentation

MRI following initial normal CT Head. Presenting with worsening personality changes, memory impairment, movement disorder and ataxia. ?demyelination.

Patient Data

Age: 45 years
Gender: Female

Symmetrical cortical grey-matter and striatal hyperintensity on DWI, with abnormal restricted diffusion. 

Case Discussion

T2/FLAIR or DWI sequences demonstrate hyperintensity of the cortical grey matter and striatum. Relative sparing of the occipital lobes and cerebellum.

In combination with the clinical picture and CSF 14-3-3 protein isolation, a diagnosis of sporadic CJD was made.
