Stanford type B thoracic aortic dissection with aortic rupture during TC

Presentation

Acute chest pain.

Patient Data

Age: 73
Gender: Male

CT thoracic angiogram

The direct examination shows a modest haematoma adjacent of the aortic arch.

In the arterial phase, is appreciable dissection with intimal flap starting from the left subclavian artery to the abdominal thoracic passage. There is a haematoma adjacent to the aortic arch with signs of rupture at the proximal of the descending aorta.

In the portal phase conspicuous mediastinal haemorrhagic spreading and in the left pleural cavity. Minimum haemorrhagic effusion in the pericardium.

Case Discussion

The worst complication of aortic dissection is the aorta rupture, usually within the pericardial cavity or the left pleural cavity.
