Right foot pain.

Age: 60 years
Gender: Female
X-ray

Right foot

No fracture, periosteal reaction or malalignment. 

Nuclear medicine

One month later

Asymmetric increased uptake within the lateral midfoot on all three phases. 

CT

One month later

Stress fracture of the fourth metatarsal shaft with periosteal reaction and cortical lucency. 

Case Discussion

This case demonstrates the low sensitivity for early-stage stress injuries and further imaging is warranted in cases of persisting symptoms.
