Subcapsular hematoma

Presentation

History of frank hematuria. Radiograph performed for surveillance of renal stones.

Linear calcification following the outline of the left kidney.

There is a peripherally calcified crescentic-shaped collection encapsulating the left lower kidney in keeping with a chronic subcapsular perirenal hematoma.

Case Discussion

A history of frank hematuria was given, however no known history of blunt trauma was given. The CT shows a subcapsular perirenal collection which follows the outline of the left kidney - this is calcified and is of low attenuation indicating chronicity of the hemorrhage.
If this was a result of blunt trauma, as no other cause is evident in the study, then this subcapsular perirenal hematoma would be considered a grade 1 AAST kidney injury as there are no visible lacerations or contusions.
