Submucosal uterine fibroid

Presentation

Heavy menstrual bleeding, dysmenorrhea and excessive vaginal discharge.

Patient Data

Age: 25 years
Gender: Female

Transvaginal

ultrasound

A rounded hypoechoic lesion in the upper part of endometrial cavity with internal vascularity.

mri

A well-defined rounded soft tissue lesion is seen completely projecting into the upper part of the endometrial cavity and attached to the left fundal myometrium. It shows isointense signal on T1, hypointense signal on T2 and homogeneous isoenhancement compared to the myometrium. Features are in keeping with a pedunculated intracavitary subendometrial fibroid (FIGO 0).

Nabothian cyst is also noted.

Mild free fluid in the pelvis.

Case Discussion

Ultrasound and MRI features are in keeping with an intracavitary submucosal fibroid. It is the least common and the most symptomatic type of uterine fibroids. On MRI, it is indenting the myometrium, isointense to the myometrium not the endometrium and has a low signal differentiating it from endometrial polyp.
