Superficial femoral artery pseudoaneurysm

Case contributed by Julio Javier González
Diagnosis almost certain

Presentation

Patient with a history of chronic kidney disease. One week ago, the patient underwent placement of a hemodialysis catheter via a transfemoral approach. The attempt was unsuccessful. After the procedure, the patient developed a pulsating lesion in the proximal one-third of the femoral artery.

Patient Data

Age: 40 years
Gender: Male

Swelling on proximal part of the left thigh.

A lesion connected to the superficial femoral vein in the proximal third is observed with liquid characteristics, inflow and outflow. On colour Doppler imaging, there is a "yin-yang" pattern.

Case Discussion

The history of puncture of the vasculature associated with the pulsatile mass and the finding of the lesion with a response to colour Doppler with a yin-yang pattern are highly suggestive of pseudoaneurysm. The patient will be taken to the operating room for repairs.
