Presentation

Instability and shoulder abduction weakness

Patient Data

Age: 35 years
Gender: Male

Delamination tear of the supraspinatus articular surface fibres, which are retracted to the level of the acromion tip. The tear involves the entire supraspinatus width. No full thickness component.

The anteroinferior labrum is deficient with chondrolabral separation and intact periosteum, in keeping with a Perthes lesion. Shallow Hill Sachs defect. Anterior band HAGL.

Case Discussion

Delamination rotator cuff tendon tears are also called cleavage tears, and refer to partial thickness tears, often the articular surface with retraction, with intact bursal surface fibres. MRI arthrograms can make articular surface tears easier to identify.

Anterior glenohumeral dislocation will injury the anteroinferior labrum and this can be associated with capsular injuries, such as humeral avulsion of the inferior glenohumeral ligaments (HAGLs).
