Trichobezoar with gastric perforation

Presentation

Abdominal distention and pain.

Patient Data

Age: 20 years
Gender: Female

There is a large heterogenous gastric intraluminal mass with several foci of gas density, extending from the fundus to the pylorus. The wall of the stomach is thickened, and at the anterior fundal wall, there is a focal defect suggestive of perforation. Marked pneumoperitoneum, causing elevation of the diaphragm and mild flattening of the IVC, consistent with tension pneumoperitoneum.

Moderate ascites.

Mild bilateral pleural effusion and basal atelectasis.

Conclusion: these features suggest a trichobezoar with perforation and tension pneumoperitoneum.

Case Discussion
