Presentation

2-week history of lower abdominal pelvic pain and discharge.

Patient Data

Age: 30 years
Gender: Female

Right ovary: 4.0 x 2.4 x 2.2 cm.

Left ovary: 5.6 x 4.4 x 5.3 cm.

Thick-walled left ovarian cyst with echogenic fluid suspicious for tubo-ovarian complex.

Case Discussion

This is a case of tubo-ovarian abscess (TOA). The patient presented with acute onset abdominal pain with fever and chills. Pelvic examination revealed left adnexa tenderness with purulent cervical discharge. A transvaginal ultrasound revealed a complex left ovarian cyst with echogenic fluid. The patient was treated with IV antibiotics for 36 hours and discharged with oral medication for an additional eight days.
