Tubular ectasia of the epididymis with dilatational deferentopathia
Presentation
Chronic right testicular pain with recent exacerbation.
Patient Data
Epididymis and mediastinum
The body and tail of the right epididymis and mediastinum testis show multiloculated cystic changes. The vas deferens appears dilated. The right epidydimal head shows a tiny cyst, measuring about 2 x 2 mm.
Findings are in keeping with tubular ectasia of the rete testis and epididymis, with possible dilatation deferentopathia.
Case Discussion
Tubular ectasia of the rete testis / epididymis is a benign condition that results from dilation of the seminiferous tubules. It is usually asymptomatic.
It results from complete or partial obstruction of the distal seminiferous ducts by a mass or cystic lesion such as a large epididymal head cyst or large spermatocele, or alternatively, from an inflammatory/infectious process such as chronic epididymitis.
Dilatation deferentopathia is a rare finding and is characterized by dilatation of the vas deferens with a thin sclerotic wall. Various etiologies are responsible for this condition, including congenital and acquired causes. However, the pathophysiology and clinical implications are not well understood.
