Presentation

Absence of testes in the scrotal sacs.

Patient Data

Age: 30 years
Gender: Male

Bilateral pelvic oval-shaped structures are seen along the lateral aspect of the urinary bladder measuring on the right side about 2 x 1.2 x 2.3 cm while the left one measures about 1.6 x 1.7 x 2.4 cm along maximal AP, TS & CC dimensions respectively.

They elicit bright T2/STIR and low T1 signals with restricted diffusion being of bright signal in the DWIs and low signal in the ADC map.

Left inguinoscrotal fatty containing hernia is seen.

Case Discussion

The testes develop at week 21 in utero and then migrate towards the inguinal canal and are complete around 30 weeks.

Undescended testis is usually diagnosed by clinical examination.

The testes usually appear as hyperintense structures on Diffusion-weighted imaging, thus used to detect them intra-abdominally.

Complications of undescended testes: testicular atrophy, infertility, and malignant transformation.
