Presentation

Trauma and neck pain.

Patient Data

Age: 25 years
Gender: Male
mri

Mild anterior displacement of C6 over the C7 vertebra is noted.

The inferior articular facet of C6 is located anterior to the superior articular facet of the C7 vertebra on the left side. Mild edema is seen in the superior articular facet of the C7 vertebra.

In the axial T2WI image, hyperintense joint fluid is absent on the left side at the C6-C7 level ( equivalent to naked facet or inverted hamburger sign seen on CT scan).

No spinal cord injury was evident.

Impression: left-sided (unilateral) facet dislocation at C6-C7 level resulting in grade I spondylolisthesis of C6 over C7 vertebra.

Case Discussion

Unilateral facet dislocation is a stable type of spinal injury where the inferior articular facet of the vertebra above moves over the superior articular facet of the vertebra below and becomes locked. It results in anterolisthesis of the affected vertebra.
