4 mm proximal left ureteric calculus. Moderate left hydronephrosis. Mild mural thickening of the left renal pelvis. Large volume of low-density fluid through the left retroperitoneum, displacing the left kidney anteriorly, and extending into the extraperitoneal pelvis.
Case Discussion
The large left retroperitoneal fluid collection is most likely a urinoma from forniceal rupture in the setting of an obstructing left proximal ureteric calculus. Renal excretory phase CT would confirm the diagnosis if contrast collects within the fluid collection.
