Acute leukoencephalopathy with restricted diffusion (ALERD)

Presentation

Fever with seizures

Patient Data

Age: 3 years
Gender: Female

Extensive diffusion restriction is observed involving the bilateral subcortical and deep white matter with sparing of the cortex.

T2/FLAIR/DWI hyperintensities are also observed involving the bilateral basal ganglia with no obvious diffusion restriction.

Case Discussion

The imaging pattern on diffusion-weighted images is called the 'bright tree' appearance, and the findings are pathognomonic for acute leukoencephalopathy with restricted diffusion (ALERD).
