Diagnosis almost certain

Presentation

Left pelvic pain. Had surgery 2 years ago for upterine leiomyoma and left ovarian cyst (pathological report not available).

Patient Data

Age: 30 years
Gender: Female
mri

There is a large tubular cystic structure with uniformly smooth and thin wall, measuring 12 x 11 x 6 cm, interposed between the left ovary and uterus with incomplete folds, eliciting a low signal on T1, high signal on T2 with enhancement of the mucosal plicae and the tubal wall, in keeping with a hydrosalpinx.

Right ovarian cyst measuring 4.3 x 3.6 x 3.3 cm of low signal on T1, high signal on T2 with a sediment of high signal on T1 and low signal on T2 "shading sign", in keeping with a hemorrhagic cyst. Hemorrhagic effusion in Douglas pouch.

Small intramural leiomyoma of anterior corporeal location (<50% intramural) Figo 6.

A small umbilical hernia is noted. 

Case Discussion

MRI features of a left hydrosalpinx in a patient with a history of surgery for uterine leiomyoma and left ovarian cyst. The right hemorrhagic ovarian cyst was probably an incidental finding.
