Presentation
Large right posterior upper thigh mass
Patient Data
There is a well defined mass between the distal fibers of the right gluteus maximus muscle measuring about 6 x 10 x 9 cm (AP/TR/CC) which appears isointense to the muscle in T1W and slightly hyperintense than the muscle on T2W and in the T2 fat sat appears heterogenous. It shows areas of hemorrhage as well as avid heterogenous enhancement.
Surrounding fat and muscle are clear.
The visualized bone appears normal.
There is intramuscular lipoma seen in the right gluteus maximus muscle measuring about 1.4 x 2.6 cm.
No obvious inguinal lymphadenopathy.
Case Discussion
Excisional biopsy was performed. Histology: right thigh mass consistent with superficial CD34 positive fibroblastic tumor.
Superficial CD34-positive fibroblastic tumor is a low-grade mesenchymal neoplasm of superficial soft tissues characterized by fascicles of spindle to epithelioid cells displaying nuclear pleomorphism and strong diffuse CD34 immunoreactivity.
