Presentation
Fever, vomiting and jaundice since seven days with perturbated serum liver function tests (elevated AST, ALT, hyperbilirubinemia).
Patient Data
hepatosplenomegaly
marked gallbladder wall thickening with no diffusion restriction (non-suppurative mural edema)
non-dilated intrahepatic and common bile ducts
Lab tests results (hepatitis A virus serology): elevated anti-HAV IgM indicating active hepatitis A infection
Case Discussion
Proven viral hepatitis A (see above).
The gallbladder wall thickening is more commonly associated with hepatitis A.
Hepatomegaly is thought to be the most sensitive sign of acute viral hepatitis while mild splenomegaly occurs in only 15 to 20% of patients.
