Congenital diaphragmatic hernia (Bochdalek) - antenatal

Diagnosis almost certain

Presentation

G1P0 at 28 weeks gestation.

Patient Data

Age: 30 years
Gender: Female

The fetal biometry was at 28 weeks + 2 days.

Posterior defect of the left hemidiaphragm well-demonstrated on longitudinal sections with intrathoracic herniation of the stomach which is located at the same level as the heart on transverse sections, displacing the heart to the right.

No other morphological fetal anomalies were seen.

Case Discussion

Ultrasound features most consistent with a congenital diaphragmatic hernia, most likely Bochdalek type, which is the commonest type. They occur posteriorly and are due to a defect in the posterior attachment of the diaphragm when there is a failure of pleuroperitoneal membrane closure in utero.
