Gallstone pancreatitis post cholecystectomy

Case contributed by Bahman Rasuli
Diagnosis certain

Presentation

Cholecystectomy three weeks ago. Now referred with abdominal pain and jaundice.

Patient Data

Age: 50 years
Gender: Male

Three low signal void stones at distal CBD are associated with upstream dilatation of extrahepatic and intrahepatic biliary ducts ( choledocholithiasis ).

High T2 signals due to edema with infiltration of the peri pancreatic fat planes as well as peri-splenic, peri hepatic and both para renal spaces due to pancreatitis.

The gallbladder is not seen.

Case Discussion

Findings are compatible with gall stone pancreatitis post-cholecystectomy because of unexpected retained or residual gall stones in the common bile duct in our patient after cholecystectomy three weeks ago.
