Giant cell tumor of the tendon sheath - 2nd and 3rd toes

Gradual swelling of the 2nd and 3rd toes from 2 years ago without a history of trauma.

Patient Data

Age: 30 years
Gender: Male

Second and third toes soft tissue swelling with increased density is present.

Well defined lytic lesions also are seen at the 2nd and 3rd toes proximal phalanges.

Pathologic fracture also is seen at the 3rd toe proximal phalanx base.

Well defined lobulated hypointense ( on T1/T2 sequence ) soft tissue mass lesions encircling the second and third toes flexor tendons, extending to the lateral extensor tendons that start from the level of 2nd and 3rd metatarsal bones head down to the distal phalanges in planter side.

The mass causing to cortical erosions of the 2nd and 3rd toe phalangeal bones and abnormal bone marrow signals(low on T1/T2) at proximal phalanxes.

Case Discussion

In the recent case, we can see the aggressive behavior of the tumor with the involvement of soft tissue and bone.

Biopsy showed a giant cell tumor.

Giant cell tumor at such a location is usually non-aggressive and needs regular follow up to detect local recurrence.
