Pelvic pain.
Patient Data



Large well-circumscribed lobulated pelvic soft tissue mass arising from the left ovary. It displays an iso signal to the myometrium on T1 and an intermediate to low signal on T2 with central high signal areas representing most likely areas of cystic degeneration +/- edema. Moderate heterogeneous enhancement is noted on postcontrast sequences.
Normal appearance of the uterus. No ascites or pelvic lymphadenopathy.
Case Discussion
MRI features of an ovarian mass histologically proven as a fibrothecoma.
Fibrothecomas are considered the most common benign solid ovarian tumor. It usually occurs in adult women, with ~66% in postmenopausal women. Larger tumors may be associated with ascites or even Meigs syndrome which may complicate ~1% of cases.
