Presentation

Farming accident - tree vs tractor.

Patient Data

Age: 50 years

>75% fiber disruption (defect measuring up to 2 cm transverse and 4-5 cm craniocaudal) of the distal muscle belly extending to the distal myotendinous junction with torn fibers folded distally; the distal free tendon is intact and attached to the proximal humerus but is mildly high signal and edematous. Adjacent fluid and hematoma.

Subcutaneous edema in the anterior chest wall subcutaneous tissues. Myofascia edema along the anterior surface of both the short head of the biceps and anterior deltoid muscle bellies.

Case Discussion

This is an example of a non-insertional pectoralis major muscle injury with the torn fibers primarily in the muscle belly but extending into the myotendinous junction
