Acute traumatic plantar hyperflexion event with deep posterior ankle pain worsened by forced plantar flexion. Forced dorsiflexion is also painful.

Prominent Stieda process with modest bone edema. There is also a small dorsal tibial osteophyte with mild joint effusion and flexor hallucis longus tenosynovitis.
Otherwise normal.
Case Discussion
Bone marrow edema in the Stieda process (elongation of the lateral tubercle of the posterior process of the talus) and the adjacent talus correlate with posterior ankle impingement syndrome.
