Presentation

Known case of hyperthyroidism, complaining of painless proptosis and limitation of eye movement.

Patient Data

Age: 65 years
Gender: Male

Minimal bilateral proptosis is noted.

Bilateral symmetrical enlargement with T2 hyperintensity and homogenous enhancement, of inferior rectus and medial rectus extraocular muscle bellies with sparing of their tendinous insertions (Coca-Cola bottle appearance).

Case Discussion

Features are consistent with thyroid-associated orbitopathy, however, it should be differentiated from orbital pseudotumor.
