Salt and pepper sign (Sjögren syndrome)

The salt and pepper sign has been used to describe the MRI appearance of the parotid gland in Sjögren syndrome due to combination of punctate regions of calcification (pepper) and fatty replacement (salt) 1.

A similar appearance may be seen in HIV-associated salivary gland disease due to mixed solid and cystic lesions that enlarge the parotid glands 2.
