The Johansson classification of periprosthetic hip fractures was one of the first classification systems proposed and is based on the anatomical location of the fracture 3

The Johansson classification, published in 1981, has been surpassed by the Vancouver and unified classification systems 3.

This classification is based on the fracture level in relation to the prosthesis 1-3:

  • type I: fracture proximal to the tip of the prosthesis with the stem still in contact with the medullary canal

  • type II: fracture extends distal to the tip of the prosthesis, with dislodgement of the stem from the medullary canal of the distal fragment

  • type III: fracture entirely distal to the tip of the prosthesis
